What the red skull means
The red skull at the top of the screen is Sin. Sin is the documented score used for next round’s killer selection, subject to participation settings. It is not your health bar, your kill count, or a rank.

Research notes
I heard a video say it plainly around the one-minute mark: the skull is the points, and the player with the most points is the killer next game. Ties are random. That matched the Sin page on the mechanics wiki, which is why I trust it. Before that I had assumed it was a bleed icon. It is not. The bleed bar is a separate meter on the dying and hooked states, described on the mechanics page.
How Sin moves
Joining a match is +1. Finishing a generator is +1. Escaping is +3. Among players participating in killer selection (killer chance enabled), the documented rule selects the highest Sin for killer. The cited video describes random selection for ties. If several players tie, the wiki says the choice is random. A shop offer of 15 Sin for 150 Robux is listed, which is 0.1 Sin per Robux. I have not opened that shop tile myself, so treat the Robux line as the wiki’s price, not a receipt.
The Sin system is dated to 18 Jul 2025 on the updates page, the same day The Abysswalker arrived. Whether the skull is only visible in the lobby is unconfirmed. I am not cropping a HUD I do not have a still of.
